The following insurances are accepted: Aetna, Ambetter/Buckeye, Anthem/Blue Cross Blue Shield, CareSource, Cigna, Humana, Molina, UMR, and United Healthcare. It is your responsibility and in your best interest to contact your insurance provider to verify your benefits. Once an appointment is scheduled, you will be expected to pay for it unless you provide 48 hours of advanced notice of cancellation.
